Register at left, download a registration form, phone 828-654-8754, send a letter to NC Mental Hope, P.O. Box 5504, Asheville, NC 28813, or just email david@ncmentalhope.org with the following information (where appropriate): Organization or team name, contact person, contact person's phone and email and a mailing address Gold Level Sponsors receive a minimum 24 x 24-inch sign at the hole of their choice on a first-come, first-served basis, their organization’s name imprinted on the event T-Shirt, and one team registration in addition to other recognition noted below. Silver level Sponsors will receive a 12” x 24” sign and their names imprinted on the event T-shirt in addition to other recognition noted below. Bronze Level Sponsors receive a 12” x 12” sign. Team Registration: Registration for teams of 4 is $200 (included in the Gold Sponsorship level). Each team member will receive an Event T-shirt and each team will receive a Team Photo. All sponsors and teams will receive continued recognition on the ncmentalhope.org and mentalhopeminigolf.org websites and a certificate of gratitude suitable for framing.
